Please include your full name, date of birth and a daytime number where you can be contacted. Please state the reason for the appointment (OB/Gyn/annual), the doctor you would like to see and an approximate time for the appointment.  Please request as much in advance as possible.  Same week appointments should be made directly through the receptionists by phone.
